Lost or Stolen Passport
Your passport is the property of the United States government; if it is lost or stolen you must immediately report it to the Department of State.
If a passport is reported lost or stolen and later recovered, you must also report this to the Department of State and return the passport as outlined here.
Once a passport is reported lost or stolen, it is no longer valid for travel -- if you later locate your passport, you must not attempt to use it for travel.

Replace a Lost or Stolen Passport
To replace a lost or stolen passport you must follow the steps for applying for a first-time passport and additionally submit form DS-64 - Statement Regarding a Lost or Stolen Passport.

Lost or Stolen Passport Abroad
Few things are more frustrating than having your passport lost or stolen while traveling abroad. Rest assured, however that most U.S. consulates have well established procedures to assist U.S. citizens in this situation.
If your passport is lost or stolen while traveling abroad you must contact the nearest U.S. embassy or consulate for assistance from their American Citizen Services unit. In most cases the U.S. consulate will issue a temporary emergency travel document or 1-year limited validity passport which will allow you to return to the United States, where you can then reapply for a full-validity passport. Full-validity 10 year passports are no longer issued onsite at U.S. consulates.

Any supporting documentation you can gather to prove your identity in the absence of your passport will expedite your application for a temporary replacement passport / emergency travel document. SOS recommends that travelers should always have a color scanned copy of your passport with you or stored online where you can access it remotely.

Report a Lost or Stolen Passport (Report Only)
Complete form DS-64 as outlined on the Department of State website. To replace the lost passport at a later date, you will need to complete DS-11 and follow all of the same requirements as applying for a first-time passport.
