Name Change / Corrections
There are several reasons why you might need your name changed or corrected on your U.S. Passport. The most common is a name change due to marriage or divorce. Additionally, the Department of State / Passport Agency does occasionally make typing or other errors when issuing passports.
Name Change Due to Marriage / Court Order - Passport Issued Less than One (1) Year Ago
If your passport was issued less than one (1) year ago, you can easily obtain a new passport reflecting your new name.

Name Change Due to Marriage / Court Order - Passport Issued More than One (1) Year Ago
If your passport was issued more than one (1) year ago, you must follow the instructions for renewing a passport. Please be sure to include an original or certified copy of your marriage certificate or the government-issued document evidencing your legal name change under federal or state law.
Correction of Printing Error (Spelling Error / Incorrect Date of Birth)
If your passport contains a spelling error or incorrect date of birth, you can apply to have it corrected at no fee any time while the passport is valid. You should always check a new passport for accuracy immediately upon receipt.
